Transitional Care Services

A Patient-Centered Approach

The Transitional Care Unit (TCU), offers spacious and comfortably furnished private and semi-private rooms. These rooms are equipped with a private telephone, cable TV and access to wireless Internet (Wi-Fi).

Rehabilitation

Our rehabilitation program is tailored to each individual. Therapists are available 6 days a week to provide one-on-one sessions.

Physical Therapy

Improves mobility and ambulation by addressing pain, balance, strength, flexibility and coordination.

Nursing

A dedicated team of registered nurses, licensed practical nurses, and nursing assistants coordinate your care and services with other members of the interdisciplinary team. Licensed nurses provide oversight, medication management, pain management, and wound management. The provision of all nursing cares and services provided is in accordance with the individualized plan of care.

Occupational Therapy

Promotes the highest level of independence by addressing safety, cognition, endurance and upper body strength/range of motion. Daily activities include bathing, dressing and meal preparation.

Speech Therapy

Specializes in disorders that involve speech, language, voice, cognition and swallowing.

The interdisciplinary team includes: clinical manager, social worker, dietitian, rehabilitation therapist, nurse practitioner, physician and you.
