Long-Term & Memory Care

The Southview Acres Health Care Center team of dedicated professionals will listen and support your nursing choices while providing skilled care 24 hours a day.

We offer an individualized plan of care designed to promote independence, engagement in social opportunities and supportive care.

For peace of mind, Southview Acres also has a controlled access memory care unit. Staff working in this area are specially trained in the care of persons with dementia.

Those living in our memory care can enjoy:

Our partners include experienced physicians and nurse practitioners from HealthPartners, HealthEast, and Aeris Medical Group to provide medical management.

Southview Acres Health Care Center offers a variety of programs and services to enhance the lives of those in our care.

Social Services

Our team of social workers help ease the transition of life at Southview Acres. With compassion and dedication, our social workers assist with all aspects of care coordination, including referrals for specialized service needs, planning for returning home and advocating for the resident.

Therapeutic Recreation

Therapeutic Recreation is more than offering entertainment. It provides interactions that can improve or maintain a resident’s physical, mental, emotional, social and spiritual well-being. A variety of opportunities for persons to engage in meaningful and purposeful programs are provided. Programs offered range from one-to-one visits to large group. Programs may include exercise/fitness, educational programs, music, games and socials; outdoor programs such as music in the courtyard, barbeques, walking club and bocce ball; community programs such as Veterans Club, Quilting Club and more!

Deaf and Hard-of-Hearing Persons

Free sign language and oral interpreters, and other services are available to deaf and hard-of-hearing persons. Ask us for help or contact our Director of Social Services at 651-554-9500. 

Spirituality

Our house chaplain, community churches and volunteers provide weekly worship services, bible studies, prayer circles and one to one visits. We embrace all faith traditions.

End-of-Life Care

Whether considering palliative care, comfort care or hospice care, our team can help to identify wishes and goals for end-of-life care. We collaborate with other healthcare professionals to meet you wherever you are on your journey.

Volunteers

Volunteers are a vital part of Southview Acres, as they offer their time and talents to serve our residents. Volunteers provide companionship, assist with a variety of activities and provide spiritual care. Southview Acres partners with community schools, churches and local youth groups to engage our residents.

The interdisciplinary team includes: clinical manager, social worker, dietitian, rehabilitation therapist, nurse practitioner, physician and you.
